National Patient Survey

 

We will be asking all patients attending the surgery throughout a week in February to complete the National Survey on the Practice.

 

Patients will be given a questionnaire with a prepaid reply envelope to post the completed questionnaires to Bolton Council for analysis.

 

We would like to thank patients in advance for completing the survey and will post the details of the response on our website, newsletter and surgery notice boards.

 

Nurses Appointments

We are now able to offer patients nursing appointments from 7.00am every Wednesday morning and until 7.00pm on Thursday evening.

Nursing appointments are now available online. 

 

The appointment system will show what is available, for example a routine blood pressure check. If you need something else, or several different things, please contact the Surgery, so we may ask you what you need and make you an appointment of the correct length.

Types available are children's vaccinations, contraceptive injections, HPV vaccination against cervical cancer, BP checks, diabetes reviews (make sure you book your blood test in advance).  

For contraceptive pill checks please ring for an appointment so your prescription can be signed by a Dr before you attend.

MEDICAL EXAMINATIONS

If you require a medical for your job, insurance or driving licence we are able to offer appointments outside the normal surgery times to complete any tests and paperwork.  This service is available for our patients or if you are registered elsewhere. Telephone for details. 

PRIVATE LETTER AND FORMS

Any requests for letter or forms (deemed as Non-NHS) that need to be completed by a doctor will incur a charge, ask the receptionist for details.

Family Planning

The nurses and doctors provide a range of contraceptive services and advice.  For your convenience this is available every day in normal appointments.

The 'morning-after pill' must be taken within 72 hours of unprotected intercourse (but it is most effective if taken in the first 24 hours).  If you inform the receptionist, you will be dealt with promptly.

Other contraception such as coil fitting and implants are available from the surgery.    Please book a normal appointment with a doctor to discuss the options. When requesting any appointment for these kind of contraception service please inform the receptionist which service you require to ensure the doctor has the correct amount of time to carry out the procedure.  Some procedures are only done by a specific doctor.  

Coil fitting appointments are available on Tuesday afternoons. 

Diabetic Care

Our nurses offer full diabetic care, and for your convenience this may be booked on any day.  Please mention when booking that it is for a diabetes review and you will be given a longer appointment.

The chiropodist and dietician services provided by the PCT are now offered separately to provide Diabetic Screening. 

Digital Retinal Screening eye checks are offered from the Royal Bolton Hospital Eye Department.

Maternity

All the doctors are on the local obstetric list and provide maternity services. We work closely with a team of midwives from the Royal Bolton Hospital who are based at Oxford Grove.

Cryocautery and Minor Surgery

The doctors will perform minor surgery that they feel is appropriate to general practice, this includes cryocautery (freezing) minor skin lesions and injecting joints. Please speak to your doctor during a normal appointment and a convenient time will be arranged, some procedures are only done by a specific doctor.

Child Health Checks

The practice provides Children's Development Checks in conjunction with the health visitor. You will be sent a card when these are due: please make an appointment as necessary.

All children will be called for vaccination through the computer system run by the Bolton Primary Care Trust.   An appointment should be made with one of the nurses.  If you need to re-arrange the time - just ring reception and we will find a convenient time for you.  If your child has a minor illness when the vaccination is due, it is best to re-arrange if there is a high temperature.

If you have any worries about your child's development, please speak to the doctor during a normal appointment or contact the health visitor at Avondale Health Centre on 463400.  Details of Well Baby Clinics can be obtained from the Health Visitors at Avondale on 463400.

Medical Examinations

The doctors undertake private medical examinations for insurance companies, HGV, PSV and for employers.  These can be arranged outside normal surgery times.  Please explain what type of medical you require to the receptionist to organize a suitable time.  A charge is made for these services; a list of fees is available in reception.  This service is available to our patients and also if you are registered elsewhere.

Phlebotomy (Blood Tests)

The practice has blood test appointments daily.  A phlebotomist comes to the surgery daily and will perform routine blood tests as requested by the doctors. This is available both morning and afternoons by appointment.
