National Patient Survey

 

We will be asking all patients attending the surgery throughout a week in February to complete the National Survey on the Practice.

 

Patients will be given a questionnaire with a prepaid reply envelope to post the completed questionnaires to Bolton Council for analysis.

 

We would like to thank patients in advance for completing the survey and will post the details of the response on our website, newsletter and surgery notice boards.

 

Nurses Appointments

We are now able to offer patients nursing appointments from 7.00am every Wednesday morning and until 7.00pm on Thursday evening.

Nursing appointments are now available online. 

 

The appointment system will show what is available, for example a routine blood pressure check. If you need something else, or several different things, please contact the Surgery, so we may ask you what you need and make you an appointment of the correct length.

Types available are children's vaccinations, contraceptive injections, HPV vaccination against cervical cancer, BP checks, diabetes reviews (make sure you book your blood test in advance).  

For contraceptive pill checks please ring for an appointment so your prescription can be signed by a Dr before you attend.

MEDICAL EXAMINATIONS

If you require a medical for your job, insurance or driving licence we are able to offer appointments outside the normal surgery times to complete any tests and paperwork.  This service is available for our patients or if you are registered elsewhere. Telephone for details. 

PRIVATE LETTER AND FORMS

Any requests for letter or forms (deemed as Non-NHS) that need to be completed by a doctor will incur a charge, ask the receptionist for details.

New Patients
Non-English Speakers

We are currently accepting patients in most areas of Heaton, Lostock, Ladybridge and Smithills we extend towards Horwich, Westhoughton, Halliwell and Astley Bridge.

Patients wishing to register with the practice should collect the necessary forms from reception or the forms are also available online to print off or email when completed.

We would also like you to fill in a brief medical record form. When the forms are received, appointments will be made with a practice nurse and doctor if necessary.

In all cases a patient registered with the practice can see any of the doctors by appointment.

New Patients

All new patients over the age of 15 are invited to attend the surgery within a month of joining to have a general health check with the practice nurse. This includes height, weight and blood pressure checks and allows us to summarise your medical history while awaiting the arrival of your notes from your previous doctor. Please try to bring details of past illness, vaccinations and smear history, where applicable, and bring a sample of urine for testing. If you are on any repeat medication, please also make an appointment to see a doctor.


Pre-Registration with the practice

If you wish to pre-register click on the link below to open the form. When you have completed all of the details, click on the "Send" button to mail your form to us. Please read the information to the right of the Pre-registration form before completing.  When you visit the surgery for the first time you will be asked to sign the form to confirm that the details are correct.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us.

Note that by sending the form you will be transmitting information about yourself across the Internet you should be confident your computer and internet connection are secure to protect your privacy. We take your confidentiality seriously and all our computer systems meet or exceed the current Data Standards in the NHS.


Alternatively you may print off a registration form, fill it out and bring it to the practice to register and book a new patient check.
