New Patients

We are currently accepting patients within the practice area. Please check your postcode on our catchment area map.

Patients wishing to register with the practice should collect the necessary forms from reception or the forms are also available below to print off or email when completed. If you cannot provide us with your NHS number you must bring some form of photo ID to prove your identity, for example your driving licence, passport or student card.

We would also like you to fill in a brief medical record form. When the forms are received, appointments will be made with a practice nurse and doctor if necessary.

In all cases, a patient registered with the practice can see any of the doctors by appointment.

All new patients over the age of 40 are invited to attend the surgery within a month of joining to have a general health check with the practice nurse. This includes height, weight and blood pressure checks and allows us to summarise your medical history while awaiting the arrival of your notes from your previous doctor. Please try to bring details of past illness, vaccinations and smear history, where applicable, and bring a sample of urine for testing. If you are on any repeat medication, please also make an appointment to see a doctor.

Pre-Registration with the practice

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us.

Alternatively, you may print off a registration form, fill it out and bring it to the practice to register and book a new patient check.
